All formal requests to see general records, personal information or to change personal information must be submitted to the township through an application form. The form must be returned to the Township Clerk with an application fee of $5.00. This fee cannot be waived, or refunded. Completed application forms and the application fee should be returned to:

Once the Township receives your request, it has 30 days to respond. Within this timeframe, the Township will provide access to the requested records, or tell you why the requested records cannot be provided.
